Please help me! 
My cardiovascular coder is out if the office this week and of course a client needs urgent coding for authorizations.
Can someone help me with the below descriptions? Again, these are for obtaining authorizations so there is no procedure report.
Separate cases:
1. Relocate tunneled hemodialysis catheter to left chest
2. Right carotid-brachial bypass with vein, and ligation of right axillary artery
3. Left arm venogram with IVUS, possible angiogram
4. Left groin biopsy of mass (unknown size)
5. cardiac coronary cta
6. MRI brain with and without contrast
7. cta arch and subclavian
