When is it appropriate to use an Lcode such as L3808 or L3807 versus using a CPT code for cast application (29075) with a Qcode for the materials?

You have to understand that casting/splinting with CPT codes are not used when pre-fabricated supplies "out of the box" are given to the patient.

The L-codes are still supplies, they also include "fitting and adjustment" as part of the code and would not allow for billing for the work CPT code for application of a splint/cast.
