I am trying to find chronic disease coding cards (cheat sheets) that include HCC information on them. We are trying to help our physicians with documentation and diagnosis specificity as it relates to HCC. The AAFP have some 2017 flash cards, the AMA offers 2018 coding cards. Has anyone used either of these? The AAFP flash cards are from 2017, are these still valid? Has there been updates in HCC codes from 2017 to 2018?

Yes, the model changes yearly. There were added ICD10 codes from 2017 to 2018.
I'm not sure about the Medicare model, but for the HHS commercial model, there were also some diagnosis codes removed. There were more adds then deletes
