Can anyone give advice on the proper code for this procedure? We have one coder saying 27438 and one stating it was a revision 27486 and another coder stating 27486-52 because he did not perform the entire 27486 code. Anyone want to comment? Looking for answers.. HELP.
Hardware Depuy oval patella
Findings lateral tracking of patella with abnormal wear of component.
Procedure: Patient brought to the operating room and placed on the operating room table. After general anesthesia was administered the left knee was examined patient of full motion well-healed incision no varus valgus instability. Positive J sign.
The left lower extremity was prepped and draped in the sterile surgical fashion patient IV antibiotic prior skin incision. A standard midline incision was made centered over the patella through the old incision. Incision made through the skin and subcutaneous tissue hemostasis was obtained with Bovie cautery. A medial parapatellar approach was performed. Fluid was normal joint fluid and sent for Gram stain and culture. The patella was everted. Patella had some abnormal wear. It was transected below the component interface. It was sized from a 34 to a 41. Guide was seated lug holes are drilled.
The 41 component was cemented in place and held with the patellar holding clamp. The knee was irrigated out with Bactisure. It was then irrigated out pulse irrigation. The extensor mechanism was repaired in a pants over vest fashion 3 #5 FiberWire suture. The extensor mechanism was then completed repair with a running #2 FiberWire suture. Subcutaneous tissues irrigated and closed deep #1 PDS suture superficially with 0 PDS sutures skin was closed with skin staples patient was placed in a sterile dressing postoperative brace extubated transferred to recovery room in stable condition.
