What would you all advise in coding ESRD (documented as such by provider) when there is no documentation for hemodialysis?
Or specifically documented as ERSD and specifically documented that the patient is not on hemodialysis

Z99.2 is used for patients with AV fistula and or dialysis dependent. A patient does not need to be on dialysis to include this code. A patient can be at level 5 due to their GFR however the physician feels the patient will quickly move into End Stage and will require dialysis

I mean, in reality a patient that has ESRD will required dialysis, is this patient newly diagnosed with ESRD and is going to start HD at a later point? Do you believe this is an error and the patient does not have ESRD because there is no supporting documentation of this condition and no treatment for it?

If there is supporting documentation, then code ESRD, no need to worry about Z99.2 if not stated.
