Capped old right ventricular lead
Replaced same pacemaker pulse generator in pacemaker pocket

What are the appropriate CPT codes : 33216 or 33207 & 33233

I do believe you will want to bill 33233 w/ either 33206, 33207 0r 33208 all depends which leads are being replaced.

Nope, they are putting the original pacer back so not 33233. Just the lead placement is billable here... 33216
