Hello,
I am very curious to know if we can code from codes that the provider has written down.
Is there a coding clinic on it?

There is a coding clinic, I believe it is 1st q 2012 that states the provider cannot use the diagnosis code in lieu of a rendered diagnosis. The diagnosis must be stated in the physicians own words and the coding should be performed by professionally trained coding staff.
