hi i just have a real quick question. do you code for office visits chronic condions that have been mentioned in the notes but are in a controlled state at the time of the visit??? I have because they have been addressed...just making sure im doing it right~!!! thanks
 
I have always coded for chronic condition if the physician addresses them during the visit. The MD may only renew Rx but still has documentation for the condition.
 
If the issue has been addressed by the provider, e.g., reviewing meds then I would count it towards the E/M level and list the diag. If the chronic condition is only mentioned and has nothing to do with this visit then it may be part of the past history only.
