Can someone please help me with a diagnosis code and procedure code to use when billing a colonoscopy for a Medicare patient.

I'm not sure what region you are in however, your Medicare contractor may have an LCD in addition to supplemental instructions for colonoscopies.
 
What kind of colonoscopy - screening(high risk or low risk) or not - were any polyps removed, if so, what technique - anything else done during the colonoscopy?
