What are the CPT Codes to bill for Community Health Worker Services?

Does the Community Health Worker have to have a NPI Number to bill for services?

Several states are billing for Community Health Workers. They are not licensed but obtain a NPI for billing purposes as the rendering provider.
My question is, does anyone know what the CPT Codes are (I found some, want to know if there are others) and if an NPI is definitely required for billing.
