We have a patient coming in who was seen in the emergency room for a concussion and needs a follow up appointmet to be cleared to go back to playing sports. When the patient comes into the office he has no symptoms. Would you code it with the concussion diagnosis code with a D or would you code Z00 follow up appointment

Since the reason for the visit is a follow up for an injury, I would use the concussion code with a D.
