What cpt would be applicable for a Covid 19 swab obtained in the office.
Would the U0001-U0005 cpt codes be applicable for commercial payers?

U codes are for labs. Physician would bill 99211 (https://www.aapc.com/blog/50839-covid-19-specimen-collection-and-testing/) and only if the visit was specifically for the specimen. If a specimen was taken during an E/M, you cannot separately bill for the specimen collection.
