If a patient is put on CPAP with oxygen, can you charge separately for the oxygen, or is the oxygen included?

What about CPAP w/ Humidifier built in?

Question about humidifiers and CPAP devices.
E0601 is the CPAP and E0562 is the HCPCS code for heated humidifier.
But what about if the CPAP has a built in heated humidifier? Should we see both codes above? Just the CPAP? Should it be an unlisted code?
Anyone have a lead on this?
