I'm hoping someone can help! I do not code for heart and vasc. but I have been helping. can someone please help me on this one? PT has state MA. The patient came into the ER - The provider did an h&p then admitted the patient.

The provider billed.

93010 (prehospital ekg)
92941-lc
93458-26-59 (denied procedure code for services rendered)
99152 (denied procedure code for services rendered)

I do know about the CMS edit but it seems every time we bill 93458 and 92941 it gets denied. They applied the mod 59 to 92941. Clear guidance would be greatly appreciated.

Bill 92941 for an acute STEMI or NSTEMI only, usually an urgent cardiac angio, PCI is needed. should not code 92941 if the procedure is done the following day or 2 days. if a diagnostic heart coronary angiogram is done the with the PCI, 92941 is the primary code. It appears you are coding correctly however different states have different rules for MA. You may just have to code 92941.
