I was wondering if anyone can explain, can or cannot the 95874 be billed with a 64615 chemodenervation code, or does it mean that absolutely not to bill with 64615 and only 64612 - 64614. Some insurance companies are paying and some are denying. If we receive payment on claims billed with the 64615 and 95874 should the monies be sent back.

Also per the 2013 CPT parenthetical note "Use 95873, 95874 in conjunction with 64612-64614" which does not include the 64615 code as one of the allowable parent codes for these add-on codes.
 
95874 and 64615

Procedure code 95874 should be billed along with procedure code 64615. Please see your errata sheet for this year. Some insurances have not updated this information on their systems but if you do an appeal it should get paid.
