I have never fully understood this, and it is high time I clarify it for myself (and for anyone else who doesn't understand but is too shy to ask the question ;))

My understanding is that ICD-9-CM Volume 3 contain procedure codes for inpatient services. How then it is that CPT codes exist in the E/M section Hospital Inpatient Services?
 
The CPT codes are for the physician to use for physician billing, the Vol 3 ICD-9 codes are strictly for facility billing of inpatient procedures. So if the physician performs a procedure on an inpatient, the physician will use CPT codes for that procedure and the facility will use the vol 3 codes. The inpatient facility does not use the E/M codes at all.
 
Ah! So there are two claims -- one for the individual and one for the hospital.

And these must correspond to the CMS-1500 and UB-04.
