Can someone help locate the appropriated Dx and CPT codes for the following:

Dx codes for:
-calcific tendonitis rotator cuff
-prepatellar bursitis
-subcutaneous hematoma for the knee or the leg
-post-op infection of the knee or the leg
-subcutaneous mass
-rotator cuff tear


CPT codes for:

-evacuation of hemotoma of the knee or the leg
-incision of hemotoma of the knee or the leg

i have to ask what this is for since these are very straight forward issues. I am willing to help but ....... what would you chose?

840.4
924.11 & 924.10 with lower leg
924.5 & 924.10 with lower leg

I am not 2 sure about some of my selections and I was unable to identify the surgical codes.
