We do electronic billing through a clearinghouse. Under the NDC/Misc. Information Tab (box 24), what are you billing it as? We received a BCBSM denial for J1050 due to remark code M123, on the claim it is noted as 150 units. Does any one know if the Depo should be in ME (milligrams) at 150 or may be as 1 Unit?

J1050 is 1mg, so your total units for Depo Provera would be 150 units.

Thank you. That is what we had on the claim and BCBS is denying it due to dosage amount.
