Prior History of urinary incontinence, now resolved.

Do I count this as 1 point or not? I am going back and forth on this. Patient hasn't been seen in a year. Last visit it was minimal. When problems are resolved, do they count towards the number of problems/diagnoses?
 
1 Dx point would be reasonable for an established problem that has improved/resolved since the last visit when it was still present, albeit minimally. Since it is resolved now, you would use the Dx code for PH of diseases of the GU system not the code for urinary incontinence (which I'm sure you know anyway).
 
Yes that is the diagnosis code that I used. (Z87.448) So what happens next year when (if) he uses it again, would i count it again as 1 point or not?
 
If the patient's problem is resolved he doesn't need to address it so 0 points. If the patient has a recurrence then it would be 2 points for an established problem that is worsening.
