A quick question regarding diagnosis selection. If a doctor indicates that the patient has asthma, but does not specify intrinsic or extrinsic is it appropriate to sign these specific diagnosis codes? The note states as below:

"Asthma; currently with exacerbation due to acute bronchitis. The patient always reports worsening of her asthma in the winter due to the cold."

I chose diagnosis code 493.92 (Asthma NOS w/acute exacerbation), the doctor chose 493.10 (Intrinsic asthma NOS).

I agree with you. There is no way to get to intrinsic asthma based on the info given.

The trail is bronchitis-asthmatic-with acute exacerbation. 493.22
or
Asthma(bronchial) -wirh acute exac 493.22
 
The code is 493.92. The provider did not state chronic or intrinsic. The coding clinics state that the provider may not use the code number as the diagnosis. The diagnosis must be rendered in the providers own words and the codes must match the narrative. Asthma with exacerbation due to acute bronchitis with give me the asthma unspecified 493.92and a code for the acute bronchitis, 466.0
