can someone tell me where to locate the guidelines (what is required) for a discharge summary? discharge summary does not require exam?

can someone tell me where to locate the guidelines (what is required) for a discharge summary? discharge summary does not require exam?
Check the cpt book for the guidelines but as far as I remember with a d/c you need to document the time spent which decides the level 99238 or 99239.
