Wiki Does Z23 need to be the diagnosis for ALL immunizations?

Good morning,

I'm looking for some insight, I have a provider who wants me to code S81.812A-Laceration of left leg as a primary diagnosis for a TD shot. I was under the impression that all vaccines should be coded with Z23? Am I able to code the TD shot 90471 and 90714 with a laceration code as the diagnosis?

I found an answer to this question if anyone else was wondering :)

Z23 is the ICD-10 code that identifies an encounter for an immunization. If the immunization is related to exposure (for example, Td vaccine administered as a part of wound care), the ICD-10 code describing the wound should be used as the primary diagnosis code for the vaccine and Z23 should be used as the secondary code.

I know that when billing the 90714 to Medicare in particular in the past they required an additional diagnosis of an injury to document a reason why the patient required the immunization.
