Can someone please tell me the diagnosis code for:

Gallbladder, Cholecystectomy:

- Cholelithiasis;
- Moderate chronic cholecystitis;
- Mild cholesterolosis

Will this be K80.10 or K80.12 or neither?

Please Help!
 
If you follow CHOLECYSTITIS > WITH > CHOLELITHIASIS - see Calculus, gallbladder, with cholecystitis

CALCULUS > GALLBLADDER > WITH > CHOLECYSTITIS K80.10 > CHRONIC K80.10

CHOLESTEROLOSIS, CHOLESTEROSIS (GALLBLADDER) K82.4

So, use both codes, K80.10, K82.4
 
Thanks, that is correct. "Excludes 1 note indicates mutually elusive codes: two conditions that cannot be reported together." There are, however, exceptions, "when the two conditions are unrelated to each other." Since we don't know the makeup of the calculus, we can't be sure if it is related to the cholesterosis or not. So, K80.10 alone will suffice.
 
the exception would not apply unless the patient had two gallbladders. For the exception to apply the two conditions must exist separately and yet whole independent of each other. such as the ankle fracture and the traumatic amputation of the lower limb.. if it is the same side then these cannot be separately reported, however if they are separate limbs then the exclude 1 exclusion can be bypassed.
