Can someone help with dx for patient comes to office for a port flush? The office report cancer or personal history of cancer as primary and Z45.2 as secondary but the insurance denied for dx is invalid. Any help would be much appreciated.

A diagnosis code can only be assigned based on the medical record - no one here can give you a valid diagnosis without have seen the physician's documentation.

But I might suggest that the history of cancer diagnosis could be incorrect. Patients do not have a port in place because of a history of cancer - ports are usually there for chemotherapy or other drugs that can't be administered through a regular IV line, so a port flush would be necessary as a part of the treatment of an active disease. So you may want to query the provider for clarification on what they were treating via the port and use that as the diagnosis. But if you've already coded a cancer diagnosis, then you just need to take it up with the payer and find out more about why they've denied it. After all, they might have just made a mistake - payers have been known to do that. :)
 
The question of why the port is being flushed comes to mind. If there was a complication with the port, that would be your diagnosis with breast cancer following. This is not a history yet, as stated above.
