would the code become a 99283 or 99284
 
I noticed you posted a couple of these. Do you have the CPT book? It says whats is required of each level.

ER always requires meet or exceed 3 of 3. so look at each code and see which one mentions problem focused history (your lowest level component of an E&M). You are trying to code too high of a level. If its Facility (UB) you will need to check your facilities guidelines for selection.
