ER consult and admission to different hospital

I have a plastic surgeon who did an ER consult in one hospital and one the same day transferred the child to a different hospital for the surgery. So the ER consult was in one place and the surgery was in another place but both took place on the same day. Child was released the following day from hospital #2. What CPT codes are allowed when filing to a medicaid managed care plan.
