Exam for participation in sports after having Covid

What diagnosis codes would be used for the examination to return to sports after having Covid. This is for pediatric patients.
 
Thank you. That is what we used, but Blue Cross and Blue Shield denied it for not covered.

From my experience BCBS will not pay for an exam that it a clearance, other than surgical, for a special circumstance ie: sports, adoption, DOT We make our patients pay for sports physicals (sports clearance) unless they have not had their wellness exam for the year, then we will do a complete wellness exam and fill out their form. We use 99429 for sports physicals only and have never had an insurance carrier reimburse for it.
