billing 64493 LT 64493 RT , 64494 RT 64494 LT & 64495 RT 64495 LT Noridian Medicare for Idaho Is not paying for 64494 or 64495 in the ASC. I billed the same codes for professional portion except used 50 modifier and was paid. Is anyone else having an issue getting paid for 64494 & 64495? Those 2 CPT codes are not on the fee schedule for facility but when I did a search there was a value. Just confused please help!
