Often times pt comes in only for flu shot. Md does not give biller, coders a dx, but he also list office visit and flu vaccine for cpt codes. In his notes he states pt only came in for flu shot. Can we bill the office visit and can the dx be v04.81
 
If the documentation only states that the patient is there for a flu shot I would not bill an office visit. I would use diagnosis V04.81 for a flu shot.

If the physician did not see the patient, but only documented that the patient was there for a flu shot, I would bill a nursing visit along with the flu shot; however some insurance companies requires a copay with the office visit and that can be more than the reimbursement. If the physician indicates that the patient was seen and treated by the nurse then I would decide whether to bill and collect a copay or just bill for the shot.
