if the physician uses chf controlled, htn controlled would you use them as diagnosis codes on the claim?

Was the condition treated or addressed during the visit, such as informing the patient to continue the medication regiment? If so, then yes, you can use it on the claim even if the condition is controlled or improving. After all, that is the condition for which the patient was being seen.
