Need some better understanding-I work in a family practice office-my Dr is
a DO. We have pt that come in with broken finger, toes,hair line fractures
that do not require casting. My question is actuallly what code do i bill
for when he states fracture care w/xray

Fracture care with x-ray, covers x-rays.
 
Fracture care

If fracture care is documented I'd use the appropriate closed fracture treatment code for the bone(s) involved. Xrays are separately reportable.

Note that most of the closed fracture codes will have a 90-day global period for follow-up visits.

Replacement casts/splints used during the global period would also be separately reportable. (Only the first cast/splint is included in the fracture care code.)

You'll have to read the fracture care codes carefully because there are many qualifiers - in addition to open vs closed, there's with or without manipulation, without or without fixation, etc.
