I notice that Medicare start denied
G0478...I added QW modifier and got denied again?
Help !!!! :(

The denial reason is CO-182 Procedure modifier was invalid on the date of service. We do have CLIA waiver, but I need to check if the number is being sent on the claim.:(

Also, if the claims were submitted before April 4th, you may need to submit them again (assuming there are no other issues). Medicare neglected to update the new G codes on the CLIA-waived list when they went into effect on Jan 1. They supposedly fixed this on April 4th.
