What denial reason are they using? Do you have a CLIA waiver? Is the number being sent on the claim?

The denial reason is CO-182 Procedure modifier was invalid on the date of service. We do have CLIA waiver, but I need to check if the number is being sent on the claim.

Also, if the claims were submitted before April 4th, you may need to submit them again (assuming there are no other issues). Medicare neglected to update the new G codes on the CLIA-waived list when they went into effect on Jan 1. They supposedly fixed this on April 4th.
