We have a patient who is in a global for a tib/fib fracture--she has fallen again and fractured the same leg in a different location. Can we now bill an E/M visit even though the patient is already in a global for the previous fracture of the same leg? If so-what modifier would we use-24?
 
What about additional fracture care?

Definitely -24 modifier
Are you also making a decision to treat a new fracture and providing a new fracture treatment?
Then you might also want -57 decision for surgery, in addition to your fracture treatment CPT.

Be prepared for a denial and for having to appeal. Be sure your MD documents thoroughly.
