Wiki headache vs migraine

Hello I hope this is the right place to ask a question- looking for help..... if the ER physician documents that pt presents with chief complaint of migraine, or pt presents with a headache and a history of migraines....... but his final impression only states headache. Which code would be appropriate? R519 or G43909?

I would either query the physician, or if you code as he documented, it would be R51.9 for headache and Z86.69 for the hx of migraine.

We need to code Headache only as the principal diagnosis code as Migraine is Exclude 1 condition with headache.
And yes, code z86.69 as well.
