We recently received denied claims from Healthnet due to coding/bundling services . The services are straight forward routine physical from ages 2 months and up. Does anyone have encountered the same issue. We are from California.. Please help.

Have you checked your diagnosis codes on the claim because based on the very last sentence of explanation code V255VDB references the line(s) billed with a secondary diagnosis in the first position.

This might not be the reason for the denial but it somewhere to start to figure out what is going on with this claim.

That what I was figuring it out. This is how it was billed in the claim form (Please see attached).
