Hi, what is the correct date of service to bill insurance for a hearing aid supply?

A patient got hearing aids which commercial insurance paid in full, but patient returned the hearing aids within the trial period. Different hearing aids were ordered and given at a later visit where we charged only V5261 hearing aid digital binaural.

What is the correct date of service for the second hearing aid V5261? Is it the date of the this second visit? Because the patient's insurance termed before the second hearing aid visit, insurance is refusing to pay for this later date of service hearing aid was given after insurance termed. Even if I use the second order date for the hearing aid it is after the insurance termed.
