Help! I work for a Cardiologist who had a spouse show up for the patients scheduled appointment, the patient is in the nursing home and could not make his appointment so the Dr. meant with the spouse instead to discuss treatment plan and prognosis. How would I code this? And what would be the diagnosis code?
