???? How would I code a 'removal of extra digit' done on a newborn in the physician office. DX 755.01
 
Polydactyly

Check your CPT book for polydactyly. You'll be referred further to the integumentary system if no bone involvement (which I'm guessing is the case if this was done in the office.)
