Dr seen a patient who came in for "ear infection" but on exam was no infection the only diagnosis Dr gave was "ear fullness"

If the patient presented with ear pain then otalgia diagnosis codes H92.01, H92.02, H92.03, H92.09 may be appropriate depending on the site.

However, if there is no pain, and no infection and the physician didn't perform any audios or remove impacted cerumen, or order any other tests to further diagnose the symptom of ear fullness, then any diagnosis code from the H93.8X series may be appropriate: H93.8X1, H93.8X2, H93.8X3, H93.8X9
