How would I code?

1. Gangrenous left foot s/p left BKA.
2. Poorly controlled diabetes with inpatient initiation of insulin therapy
3. Methicillin-sensitive Staphylococcus aureus bacteremia with amputation of source.
4. Phantom pains as a result of amputation.

I was thinking - 250.82, 707.15, 785.4 and V49.75, is this correct?
 
How about:

1. 250.72 Diabetes with peripheral circulatory disorders. Underneath this code in the notes, it says to also code the manifestations and gangrene would be:
2. 785.4 Gangrene
3. 041.12 Methicillin resistant staphylococcus aureus (MRSA) or V12.04 for history of if they are now diagnosed as MRSA free.
4. 353.6 Phantom limb (syndrome)
