High Risk Colonoscopy dx

My doctor has a claim for a patient with a personal history of breast cancer getting a screening colonoscopy. Medicare won't accept Z85.3 as a diagnosis code for G0105- what would you use? No other family history, personal history, or other usable code in the documentation.

if the screening colonoscopy without any abnormal finding ( colon polyp), the primary dx is encounter screening colonoscopy.

Why are you coding the high risk colonoscopy if the dx doesn't support it? I would query the provider if he is requesting G0105 and not G0121.

I would bill as a normal screening Z12.11 and G0121 if the patient hasn't had one in the past 10 years. Z85.3 isn't listed as an approved dx for colonoscopy.
