Patient is returning to office for follow-up on 162.3. Patient has NO clinical evidence of disease in lung. The patient has been diagnosed with colon cancer since last follow-up one year agi, but was diagnosed and treated elsewhere than our radiation oncology clinic. For the follow-up on 162.3, should the primary diagnosis code be V10.11 (Personal history of Bronchus or Lung) or V10.11/162.3 OR 162.3/V10.11. I know many carriers dislike the V codes.

If the provider is saying no clinical evidence AND the patient is not receiving ANY treatment or meds, then you must use the V codes

But if the patient is receiving ANY treatment or meds to prevent a reoccurrence of the cancer, then it is my understanding that you must code the cancer as active 162.x
