Hello,

I'm working on an Oncology office f/u note.... but am having difficulty with the HPI elements.

MR. returns for f/u of his T1, N2 squamous carcinoma of the oropharynx. He presents now to sign an informed consent and to begin his comibine chemoradiotherapy. Since his last eval, he has no new complaints and is generally feeling well. His final pathology results have come back and confirm that his tumor is HPV positive.

So far I've come up with location, severity, flip/flopping on severity or Associated signs and symptoms.... ( please note that my medicare carrier is Highmark Medicare ).

I feel like I'm missing something here but maybe I'm not....
