Good Morning,
I need your help
I received a denial from Humana with this remark description: This service is not allowed because the diagnosis code(s) are inappropriately coded. The line item denied is cpt 99213 with 25 Mod and dx G56.02, G56.01 and M65.4.
We have billed more line items but this is the only one denied.
I have read the guidelines and I can't find anything on sequencing regarding these codes.
Any help will be appreciated.

it could be the use or the right and left instead of the bilateral, it could be the use of the De Quervain's with the carpal tunnel. what was the reason for choosing these codes in this combination?
 
This pt was seen for hand and wrist pain in both hands. Let me get the bilateral dx code. Thanks so much, sometimes you look at a problem so close and for so long that you don't see the answer right there! Thanks again! I think I was more confused by the denial than to have a better look at the codes.
