I need help on coding Ortho conditions and the supplies used for that same-day visit patient please

I need help coding this patient.

I Keep on getting denials for supplies saying "CPT A6448 still denied for Missing/incomplete/invalid procedure code." even if this is coded by 99070, A6448-NU.

I told the providers to specify the supplies used when documenting so I could assign the correct codes.

Another possibility that I am thinking of is that those who put the codes in our system could have been typing the codes on different lines every time, so it turns out that A6448 is billed on its own without any procedure code, even if I coded 99070 along with it.


Please also let me know what I could have done better because no one is guiding me. I research a lot but it turns out that it is not enough.

5x year old male presented with R elbow pain x last night . (new patient)

History Of Present Illness:

Patient is having R lateral elbow pain and tenderness with movements. He states that this is new onset pain and denies any trauma to the joint. He denies any recent reparative activities and denies any sports play recently. Onset date August 12, 2025

Physical Exam:	Constitutional: NAD Eye: PERRL Neck: supple Respiratory: NAD Lymphatic: WNL Musculoskeletal: + FROM in R elbow with mild tenderness over the lateral epicondyle + mild tenderness over the R forearm with finger movement / gripping of hand Skin: Intact Extremities: Negative edema Neurological/Psychiatric: Grossly Intact

ASSESSMENT:
Diagnosis:	ICD-10 Codes: 1)M7711; Lateral epicondylitis, right elbow 2. M25521 pain in right elbow

Care Plan:

Patient declines X-rays. He is given Prednisone for inflammation and pain, and an ACE wrap in office. He is given detailed instructions on how to use the medications safely and properly. He is to use RICE therapy practices to help with the pain. He is to limit his stressful activities such as lifting / gripping. He is to follow up with this clinic or his PCP for continued care of this condition. Supportive care guidelines are given to him.
