I'm also confused and would need more information - you're asking for a CPT code - is it part of a physical exam? Or are you looking for a diagnosis code since this is the diagnosis coding thread?

If CPT - I would look for what E/M fits the documentation.
If ICD9-CM - I would look for the underlying condition - why does the patient need a foot check - diabetes, PAD, etc.?
