Dx: Cervical radiculopathy, cervical degenerative disc disease, facet syndroms.

Procedure performed: Fluoroscopically guided cervical transforaminal epidural
steroid injection at Epidurography.
Fluroscopically guided left and right medial branch
blocks at C4, C5, C6, AND C7, Interpretation of imaging
studies.

medial branch blocks are 64470-64472 for the cervical spine and transforaminal are 64479-64480.

Both are mentioned and I have a feeling its supposed to be one or the other. please compared op notes to both code sets before making your determination

Your Transformainal injection are 64479 - 64484. Keep in mind it saids "single level" not interspace. :p
 
Transforaminal injections are administered by way of the intravertebral foramen located on each side of the vertebra where the nerve root exits the spinal column. The CPT code assignments for transforaminal injections of an anesthetic agent and/or steroid are as follows:

64479, single-level injection cervical/thoracic region;

64480, each additional level cervical/thoracic region;

64483, single-level injection lumbar/sacral region; and

64484, each additional level lumbar/sacral region.
