a patient came in for a medical (Mifepristone/Mifeprex) termination. After 11 days, it was determined the process failed, so she underwent a vacuum aspiration procedure to remove the pregnancy. How do I code the second procedure? (ICD10)
 
The process failed, so the patient still had a viable pregnancy? It looks like your question concerns the diagnosis code. If there was still a viable pregnancy and she still opted to proceed with a induced abortion, it would be Z33.2. If the pregnancy was not viable and that was being treated, I would lean towards using O07.4.

Would I use the O07.4 or continue to use O02.1? Patient had a 7wk Missed abortion and the Mifepristone termination didn't work. Patient had some spotting. D&C was performed after confirming on OB US fetus had not passed.
