I need a little confirmation on billing out immunizations. We are a pediatric clinic with high volume of vaccines when it comes to the administrations it can be a very confusing.

If we have a patient that comes in for a office visit and immunizations we do the E/M level and and counsel the patient on vaccines we use 90460 for the first component for each admin and 90461 for every component after. The only question I have is if we do an oral vaccine with this is the oral vaccine included in the administration for the 90460 and 90461 or do we do a separate 90473 or 90474. Can someone please clarify this for me.

Also if it is a vaccine without counseling or office visit we use 90471 and 90472 but for the oral do we use 90473 or 90474.

90460x4units 90461x6 units
if no counsel say you had
flumist intranasal
MMR
varicella
prevnar
you could key 90473 1st of the oral or internasal vaccine the flumist then 90472 x 3 units each addtl vaccine
or if you prefer to key in a different order u could key your Oral/intranasal last and it would be like this
90471 MMR
90472 x 2 prevnar and varicella
90474 flumist I think that's correct but if your doing VFC each state / mgd mcday have their own rules. also make sure your vaccine CPT codes cross overwatch the cot code for. vaccine admin. fee if claim is more than. 6.line items
